RN Case Manager – Infectious Disease

Job Description

The RN Case Manager for Infectious Disease will coordinate medical and behavioral health services for patients in the Harm Reduction Program, focusing on those with HIV, TB, Hepatitis B and C, STIs, and syringe services. Key responsibilities include:

• Contributing to chronic illness care networks.
• Managing day-to-day and long-term patient care plans.
• Addressing health education needs of patients and families.
• Supporting chronic care patients in achieving self-management goals.

Qualifications include:

• Graduation from an accredited nursing program.
• Current Massachusetts RN licensure.
• Preferred case management experience.
• Bilingual skills in Spanish/Khmer/Russian are beneficial.
• Ability to work collaboratively in a multi-disciplinary team.
