Job Title: 2nd Shift LPN Charge Nurse

Essential Functions:

• Monitor daily resident care and perform skilled nursing tasks
• Document resident conditions and communicate with families and healthcare providers
• Ensure implementation of care plans and manage medication records

Supervisory Responsibilities:

• Assist with shift assignments and monitor staff performance
• Manage staffing and address family concerns

Minimum Requirements:

• Current LPN/LVN or RN license
• Experience with elderly care and supervisory experience preferred
• Strong documentation and relationship-building skills
