We are seeking a dedicated Occupational Therapist to join our home health care team in the Fremont/Fostoria areas. This full-time position offers flexibility and support in scheduling and documentation, allowing you to focus on patient care.

Job Responsibilities:

• Communicate with office staff to manage patient intake and paperwork.
• Maintain open communication with patients and home health agencies.
• Collaborate with Physical Therapist Assistants for patient care.
• Complete evaluations, care plans, reassessments, and discharge paperwork.
• Participate in meetings and case conferences.

Job Requirements:

• Current Occupational Therapist Licensure.
• Bachelor’s degree in Occupational Therapy.
• Ability to travel to patient locations.
• Strong communication skills.
• Home healthcare experience preferred.
