Responsibilities:

• Conduct initial nursing assessments for new admissions.
• Coordinate and revise patient care plans, ensuring continuity of care.
• Communicate patient progress to the care team.
• Maintain accurate patient care records.
• Educate families on patient needs and conditions.
• Perform on-call duties as assigned.

Qualifications:

• Current RN license in the relevant state.
• One year of nursing experience preferred; new graduates considered.
• Current CPR certification.
• Valid driver’s license and reliable transportation.
