Job Description

The Home Health Speech Therapist is responsible for delivering speech and language services to clients, adhering to the values of Compassion, Integrity, Excellence, Teamwork, and Innovation. Key responsibilities include:

• Developing and implementing speech therapy plans.
• Conducting patient assessments and evaluations.
• Educating patients, families, and team members.
• Documenting care accurately and timely.
• Participating in interdisciplinary team meetings.
• Supervising Home Health Aides as needed.
• Ensuring compliance with ethical standards and regulations.

Qualifications include a Bachelor’s degree in Speech Therapy, with a Master’s preferred, and at least one year of home health experience. An active Speech Therapist license is required.
