We have openings for Mid-shift and Weekend Social Worker positions.
Mid-shift Social Worker:
Full-time (40 hrs) 11a-7p or part-time (16-24 hrs) 4p-8p, Monday-Friday with weekend/holiday rotation.
Provide holistic care to terminally ill patients and their families in Grand Rapids, MI.
Assess needs and assist with the Plan of Care in collaboration with an Interdisciplinary Team.
Weekend Social Worker:
Part-time, working Saturdays and Sundays from 10a-6p with holiday rotation.
Similar responsibilities as the Mid-shift position.
Qualifications:
LLMSW or LMSW license required; experience in hospice or related fields preferred.
Flexible and skilled in symptom management.
