The Registered Nurse Case Manager is responsible for planning, organizing, and directing hospice care, focusing on community health education. Key responsibilities include:
Conducting comprehensive assessments of patients and families to determine hospice needs.
Developing and revising care plans based on patient evaluations and nursing diagnoses.
Administering medications and treatments as prescribed, while providing health care instructions and counseling.
Maintaining accurate clinical documentation and communicating with physicians and community health personnel.
Supervising hospice aides and participating in quality improvement activities.
