RN Case Manager

Job Description

We are seeking a Registered Nurse Case Manager to join our team, providing high-quality, compassionate care to patients in Tulare and Kings Counties. Ideal candidates will have home health experience, though training and support will be provided.

Requirements:

• 1+ year of RN experience with a valid license
• Current CPR/BLS Certification
• Valid driver’s license and auto insurance
• Comfortable driving to patient homes
• Home Health, Hospice, Infusion, or Home Care experience preferred

Responsibilities:

• Develop and implement patient Plans of Care (POC)
• Establish patient-specific goals and provide nursing services
• Oversee quality, patient-centered care in home settings
• Communicate patient status revisions with the care team
• Consult and reevaluate patient status with families and team members
• Ensure accurate and timely documentation
