We are seeking compassionate Licensed Practical Nurses (LPNs) to provide one-on-one care in patients’ homes, promoting independence through education and skilled nursing. This full-time position includes Monday to Friday shifts with weekend and holiday on-call rotations.

    • Responsibilities:

  • Coordinate patient care with RN and physician
  • Document patient conditions and care
  • Assist with daily living activities and self-care education
  • Follow safety and emergency procedures

    • Qualifications:

  • 1 year of LPN experience
  • Current LPN license and CPR certification
  • Valid driver’s license and flexible schedule
  • Strong interpersonal skills
