Job Description

This position involves comprehensive care coordination for patients at WVU Medicine, focusing on psychosocial assessments, crisis intervention, resource management, and discharge planning. The role requires collaboration with a multidisciplinary team to address complex psychosocial needs and facilitate transitions to alternate care levels.

Minimum Qualifications:

• Master’s Degree in Social Work
• LGSW/LCSW/LICSW certification in West Virginia

Preferred Qualifications:

• 1-3 years of experience

Core Responsibilities:

• Manage transition/discharge planning
• Collaborate with care teams
• Provide education and assessments
• Document interactions and plans
• Assist with financial and resource management

Occasional on-call and weekend shifts required. Strong interpersonal skills and the ability to work with diverse populations are essential.
