We are seeking a Physical Therapist with home health experience to join our dedicated team. In this role, you will provide direct patient care and collaborate with an interdisciplinary team to enhance the care experience for patients and their families.

Responsibilities:
Provide physical therapy services per physician’s plan
Initiate therapy programs and instruct caregivers
Prepare clinical summaries and participate in discharge planning
Supervise Physical Therapy Assistants
Ensure compliance with regulations
Qualifications:
Graduate of a CAPTE-approved program with current state licensure
2 years of adult physical therapy experience; home health preferred
