An experienced RN Case Manager is needed for a full-time position in Walsenburg/Trinidad, Colorado, focusing on homecare services. The role involves assessing and addressing the physical, psychological, social, and spiritual needs of patients and their families, initiating care plans, and collaborating with an interdisciplinary team.

Responsibilities:
