> REF:#6fa72562f5d10ef4The LCSW/LPC role involves providing services in three key areas: patient support, enhancing healthcare quality, and community resource efficiency. Direct services are delivered through Rural Health Clinics and include:
- Interventions to improve psychosocial functioning
- Adaptation to illness and disability
- Post-hospitalization planning
- Psychosocial assessment and evaluation
- Facilitating referrals and resource knowledge
- Communicating psychosocial needs to the interdisciplinary team
- Collaborating with administration to enhance resources for patients and families
Qualifications include a Master’s degree in Social Work or related fields, one year of post-graduate experience (preferably in healthcare), and relevant licensure. Strong communication skills and the ability to consult with various stakeholders are essential.
