LCSW/LPC Social Worker

Job Description

The LCSW/LPC role involves providing services in three key areas: patient support, enhancing healthcare quality, and community resource efficiency. Direct services are delivered through Rural Health Clinics and include:

• Interventions to improve psychosocial functioning
• Adaptation to illness and disability
• Post-hospitalization planning
• Psychosocial assessment and evaluation
• Facilitating referrals and resource knowledge
• Communicating psychosocial needs to the interdisciplinary team
• Collaborating with administration to enhance resources for patients and families

Qualifications include a Master’s degree in Social Work or related fields, one year of post-graduate experience (preferably in healthcare), and relevant licensure. Strong communication skills and the ability to consult with various stakeholders are essential.
