Registered Nurse (RN) Case Manager

Job Description

We are seeking compassionate Registered Nurses (RNs) for a full-time Case Manager role, working Monday to Friday, 8 AM to 4 PM. The position involves one-on-one patient care in home settings, focusing on developing and managing individualized Plans of Care to enhance patient independence and satisfaction.

Primary Responsibilities:

• Oversee patient care from admission to discharge, including monitoring and reassessment.
• Coordinate care with physicians and other disciplines.
• Educate patients and caregivers on care protocols and disease management.
• Manage resources and participate in care team meetings.
• Supervise Licensed Practical Nurses and ensure accurate documentation.

Qualifications:

• 2+ years in acute or post-acute care preferred.
• Active RN licensure and CPR certification required.
