Title: Primary Care Physician

Location: Indiana Region – Openings in Fort Wayne, Indianapolis, Speedway, Gary, Hammond, and South Bend.

Role Description: Seeking a Primary Care Physician to provide equitable, value-based healthcare to Medicare patients. Physicians will work at a single center with support from large care teams, focusing on quality over volume. New graduates are encouraged to apply.

Schedule: Full-Time, Monday to Friday, 8:00 AM – 5:00 PM

Responsibilities:

• Assess and diagnose patients
• Oversee primary care administration
• Prescribe medications
• Maintain electronic medical records
• Collaborate to meet health quality goals

Qualifications:

• M.D. or D.O. Graduate
• Board Certification in Internal or Family Medicine
• Active State License and DEA License
