Summary:

The Registered Nurse Patient Navigator Lead is responsible for providing health coaching, case management, and expertise in health services. This role involves monitoring cases, analyzing health data, and coordinating disease management programs while interacting with members and providers.

Responsibilities:

• Oversee integrated medical benefit programs and case management.
• Coordinate patient care across the continuum.
• Utilize tools and methodologies for effective case management.
• Identify high-risk individuals and develop risk-reduction programs.
• Monitor case management outcomes and prepare cost analyses.
• Maintain professional standards and effective communication.

Job Requirements:

• Associate’s Degree in Nursing.
• 5 to 7 years of experience.
• RN License and BLS certification required.
