> REF:#65c52b0eae8a7867The Social Worker is a key member of the interdisciplinary team responsible for assessing psychosocial symptoms in patients and families, developing outcomes to alleviate these symptoms, and providing psychoeducation, supportive counseling, and casework interventions. Key responsibilities include:
- Conducting bio-psycho-social-spiritual assessments.
- Providing individual and family counseling.
- Referring families to community resources.
- Maintaining accurate patient documentation.
- Participating in interdisciplinary care planning and team meetings.
- Offering bereavement care and group activities.
- Supervising interns and participating in training.
Qualifications include a Master’s in Social Work, at least one year of healthcare experience, and relevant state licensure.
