Homebase Medical is seeking an Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) for a part-time role focused on providing primary care to homebound older adults in the San Jose, CA area. Responsibilities include performing home visits, health education, and utilizing an EMR system for documentation. Candidates must have a PA or NP license in California, at least 2 years of primary care experience, and be bilingual in Vietnamese.
