The Admissions Nurse is responsible for planning, organizing, and directing hospice care, focusing on community health education. Key responsibilities include:
Conducting comprehensive assessments of patients and families to determine hospice needs.
Developing and initiating care plans, regularly re-evaluating patient status, and making necessary adjustments.
Providing professional nursing care, administering medications, and offering health care instructions.
Maintaining accurate clinical documentation and communicating with physicians and community health personnel.
Participating in on-call duties and continuing education for professional development.
Qualifications include an Associate’s degree in nursing, relevant experience, and current licensure.
