Job Summary:

Offering a $10,000 sign-on bonus (prorated for part-time hires), this position involves nursing assessments of clients in their homes in the Greenfield area. Responsibilities include:

• Developing and updating a plan of care (POC) in collaboration with physicians, clients, and families.
• Coordinating nursing services and assessing health needs and hazards.
• Providing intermittent nursing care, teaching, and supervision.
• Consulting and referring families to appropriate resources.

Qualifications:

• Graduate of an N.L.N. approved nursing school.
• One year of nursing experience in an acute care setting preferred.
• Valid nursing license in Massachusetts and a driver’s license.
• Basic computer skills and proficiency in English.
