Join our team as a Home Health Occupational Therapist in SE Portland, OR, where you’ll provide person-centered care to seniors, helping them achieve therapy goals and improve their quality of life.

Minimum Qualifications:

• Licensed Occupational Therapist in Oregon
• Graduate from an accredited Occupational Therapy program
• CPR Certification
• National Provider Identifier (NPI) required upon hire

Preferred Qualifications:

• Experience in Home Health
• Strong organizational skills

Responsibilities:

• Establish treatment plans based on physician orders
• Complete clinical and progress notes
• Educate patients and families on treatment goals
