Home Care RN Admission Nurse

Job Description

Adara Home Health seeks a Clinical Admissions Nurse to provide client care in line with company policies while maintaining strong relationships with referral sources. This full-time position, based in St. Cloud, offers a Monday to Friday schedule with on-call rotation.

Key Responsibilities:

• Conduct assessments and develop care plans for clients.
• Provide comprehensive nursing care, including medication administration and education.
• Coordinate care with families and other professionals.
• Participate in team meetings and maintain effective communication.

Qualifications:

• Current RN license in Minnesota with two years of healthcare experience.
• Strong organizational and communication skills.
• Knowledge of Medicare certification and OASIS documentation.
