Job Responsibilities:

• Communicate daily with office staff to manage patient intake and paperwork.
• Maintain open communication with patients and home health agencies.
• Collaborate with Physical Therapist Assistants for patient care management.
• Complete evaluations, care plans, reassessments, and discharge paperwork.
• Participate in meetings and case conferences.

Job Requirements:

• Current Occupational Therapist Licensure.
• Bachelor’s degree in Occupational Therapy.
• Ability to travel to patients’ homes within a 40-mile radius.
• Strong communication skills; home healthcare experience preferred.
