Job Description

The Ambulatory Care Clinical Pharmacist will collaborate with an interdisciplinary team to manage disease states and oversee patient care within the ECU Health System. Responsibilities include:

• Conducting disease state management and analyzing patient information.
• Formulating medication therapy problem lists and developing care plans.
• Implementing care plans in collaboration with healthcare teams and educating patients and families.
• Monitoring and following up on patient outcomes, including remote monitoring.
• Providing education to healthcare professionals and pharmacy students.

Minimum qualifications include a PharmD, two years of clinical pharmacy experience, NC pharmacist licensure, and BLS certification.
