Job Description

Fallon Health seeks a social worker for its Summit ElderCare® program, which supports individuals aged 55 and older in living safely at home. The role involves:

• Conducting initial assessments and ongoing evaluations of participants.
• Developing and updating social service plans.
• Maintaining accurate records and documentation.
• Participating in interdisciplinary team meetings.
• Advocating for participants and assisting with caregiver support programs.
