Requirements:
- Current licensure as a Nurse (RN/LPN/LVN), Respiratory Therapist (RT), or Medical Social Worker (per state guidelines).
- Previous critical care experience required; discharge planning experience preferred.
Responsibilities:
- Conduct utilization reviews and manage resources.
- Develop and implement patient-specific discharge plans.
- Coordinate with payors for reimbursement and ensure compliance with policies.
- Facilitate multi-disciplinary team meetings.
