Job Description

Position: Case Manager at a Critical Illness Recovery Hospital

Requirements:

• Current licensure as a Nurse (RN/LPN/LVN), Respiratory Therapist (RT), or Medical Social Worker (per state guidelines).
• Previous critical care experience required; discharge planning experience preferred.

Responsibilities:

• Conduct utilization reviews and manage resources.
• Develop and implement patient-specific discharge plans.
• Coordinate with payors for reimbursement and ensure compliance with policies.
• Facilitate multi-disciplinary team meetings.
