We are seeking a compassionate RN Case Manager to join our team, supporting patients and families in Meridian and surrounding counties during their end-of-life journey. This full-time position offers a Monday-Friday schedule from 8 AM to 5 PM.

Responsibilities:

• Manage patient and family caseloads through assessment and care planning
• Coordinate with interdisciplinary teams and physicians
• Conduct regular patient visits and maintain accurate records
• Supervise Home Health Aide care delivery

Qualifications:

• Graduate from an accredited nursing program
• Current RN license and CPR/First Aid certification
• Valid driver’s license and insurance required
