The Case Manager will collaborate with physicians to manage individual patient programs, ensuring effective clinical and financial outcomes for those with various disabilities. Candidates must have current state licensure and at least 3 years of healthcare experience, with preferred qualifications including rehabilitation experience and case management certification.
