Job Description

This position involves providing direct services to patients in a Behavioral Health Program. Key responsibilities include:

• Conducting psychosocial assessments and developing treatment plans with patients and a multidisciplinary team.
• Delivering behavioral health care in line with treatment plans and providing crisis intervention.
• Coordinating care with physicians and community agencies, and ensuring proper discharge planning.
• Maintaining patient documentation and participating in performance improvement activities.
