We are seeking a Licensed Speech-Language Pathologist (SLP) for a Care Coordination Program, offering a $2,500 sign-on bonus. This role allows SLPs to provide therapy services to seniors in various living settings while engaging in care coordination tasks. Responsibilities include conducting assessments, treatment planning, and wellness visits, as well as collaborating with a multidisciplinary team.
