**Position Summary:** The PT – Physical Therapist delivers direct rehabilitation care to patients based on physician referrals, adhering to relevant laws and standards.
**Education, Experience, and Licensure Requirements:**
Graduate of an accredited physical therapy program (BSPT, MSPT, or DPT).
Active state license in good standing.
Preferred one year of experience in post-acute care.
CPR certification required upon hire.

Proficient in Microsoft Office.
Collaborate with patients and families for care planning.
