Advanced Practice Provider (APP) – Rhinelander area

Job Description

We are seeking an experienced Advanced Practice Provider (NP/PA) to deliver onsite medical care in skilled nursing facilities in the Rhinelander area. This full-time position (Monday-Friday, no nights, weekends, or holidays) focuses on acute and chronic care management for adult and geriatric patients.

Key Responsibilities:

• Provide primary care and develop personalized treatment plans.
• Establish diagnoses, prescribe medications, and perform procedures.
• Document patient assessments and coordinate care with specialty providers.
• Communicate effectively with patients, families, and the clinical team.

Qualifications:

• Valid NP/PA license and DEA.
• Minimum 1 year of experience preferred.
• Strong clinical and interpersonal skills.
