Key Responsibilities:

• Conduct initial evaluations and ongoing assessments of patients’ social work needs.
• Assist with funeral planning, advanced directives, and community resources.
• Provide counseling and bereavement support.

Qualifications:

• Master’s degree in Social Work and state licensing.
• Minimum one year of healthcare experience.
• Understanding of hospice philosophy and grief counseling.
