• Position: RN Inpatient Case Manager IV
• Type: Full Time, Days

Responsibilities:

• Provide clinically-based case management for effective patient care.
• Integrate utilization management, care coordination, and transition planning.
• Collaborate with interdisciplinary teams to enhance patient outcomes and satisfaction.

Qualifications:

• Master’s degree in nursing + 5 years of relevant experience or Bachelor’s + 10 years required.
• Current NM RN licensure and 2 years of clinical nursing experience.
• National Case Management certification preferred.
