PHYSICAL THERAPIST – FMH PT BURCHARD

Job Description

Job Summary:
This position offers a $10,000 commitment award for a 3-year tenure. The primary hours are 9:30 am to 6 pm, with 10-hour shifts and some flexibility. The Physical Therapist will implement treatments to enhance patients’ functional independence, including evaluations, exercises, and pain management.

Job Responsibilities:
– Evaluate patients and develop treatment plans with input from patients and caregivers.
– Document services and consult with other disciplines.
– Design and educate on assistive devices.
– Tailor programs to meet individual patient needs.
– Conduct home/work site evaluations.
– Educate patients and families on rehabilitation techniques.
– Supervise support personnel and participate in quality improvement initiatives.

Requirements:
– Education: Doctorate or Master’s in Physical Therapy preferred; Bachelor’s with relevant experience considered.
– Licensure: Current PT licensure in Illinois and AED/BLS certification required.

Physical Requirements:
– Involves various physical activities including lifting, standing, and walking, with the ability to handle patients of all ages.
