Signature HealthCARE of Monteagle Rehab & Wellness Center is a 150-bed facility specializing in dementia and behavioral care. We aim to transform healthcare through education and empowerment. We seek a detailed-oriented MDS Coordinator who is organized and self-motivated, with bedside assessment experience. Responsibilities include collecting MDS information, participating in meetings to ensure compliance, and educating the interdisciplinary team. Candidates must be a Registered Nurse with at least three years of clinical experience and one year of MDS experience.
