Job Description

Position Summary: The role involves managing and coordinating Care Coordination services within a health system, focusing on person-centered care for patients in various settings. The position emphasizes collaboration with clinical leaders to develop protocols and ensure high-quality services.

Requirements:

• Education: Registered Nurse, BSN preferred.
• Licenses: Current New Mexico Nursing License.
• Skills: Knowledge of healthcare policies, ability to coordinate services, strong communication skills, and cultural sensitivity.
• Experience: Minimum of two years in a healthcare setting, with a focus on quality and care coordination preferred.

Supervision: Reports to Manager of Population Health and Director of Ambulatory Patient Care Services.

Environment: Involves multiple settings with potential exposure to bloodborne pathogens.

Physical Requirements: Ability to navigate care settings, perform close work, and manage stressful situations while providing excellent customer service.
