Part-time Nurse Practitioner

Job Description

We are seeking a part-time Nurse Practitioner or Physician Assistant to provide medical care to patients in a Skilled Nursing Facility in Lake Maccamaw, NC. This role involves conducting health assessments, developing care plans, managing chronic conditions, and collaborating with healthcare professionals.

Primary Responsibilities:

• Conduct comprehensive health assessments and medication reconciliation.
• Develop individualized care plans with the interdisciplinary team.
• Manage chronic and acute illnesses, order diagnostic tests, and prescribe medications.
• Provide education to residents and families on health maintenance.
• Maintain accurate documentation in electronic medical records.

Minimum Requirements:

• Current Nurse Practitioner licensure in NC and board certification.
• Experience with the geriatric population preferred.
• Strong assessment and communication skills.
• Proficiency in electronic medical record systems.
