Job Description

We are seeking an experienced advanced practice provider to support heart failure patients in Cleveland County. As a community-based, not-for-profit hospice and palliative care provider, we prioritize high-quality end-of-life care for all patients, regardless of their ability to pay.

Essential Functions:

• Provide high-quality end-of-life care and manage pain and symptoms.
• Guide patients and families on disease progression and treatment options.
• Conduct evaluations, prescribe therapies, and recommend referrals.
• Participate in team discussions and pursue educational opportunities.

Qualifications:

• Preferred cardiac or heart failure experience.
• Minimum 1 year of experience in direct care of patients with life-limiting illnesses.
