Job Description

The Chronic Care Coordinator, RN position focuses on providing evidence-based wellness promotion and self-care management for Medicaid, Medicare, and dual-eligible members with chronic conditions. Key responsibilities include:

• Assessing, planning, and coordinating healthcare services to meet members’ needs.
• Advocating for members and collaborating with the interdisciplinary care team.
