MDS Coordinator

Job Description

The MDS Coordinator ensures the accurate and timely completion of Medicare/Medicaid case-mix documents to secure appropriate reimbursement for care services. Key responsibilities include:

• Assessing health status and level of care for new admissions.
• Completing MDS Assessments and coordinating interdisciplinary participation.
• Maintaining MDS assessment schedules and ensuring thorough documentation.
• Conducting resident care conferences and assisting in care plan formulation.
• Managing level of care changes and ensuring compliance with regulations.

Qualifications include a current RN license, preferred long-term care experience, and strong communication skills. Physical demands involve regular sitting, standing, and lifting up to 25 pounds.
