Job Summary:

Offering a $10,000 sign-on bonus (prorated for part-time hires), this position involves nursing assessments of clients in their homes. Responsibilities include developing and updating a plan of care (POC) in collaboration with physicians, clients, and families, as well as providing coordinated nursing services. The role requires evaluating health needs, delivering intermittent nursing care, and providing education and supervision. Referrals to appropriate community resources are also part of the job.

• Education: Graduate of an N.L.N. approved nursing school.
• Experience: One year in acute care or community health nursing preferred.
• Licensure: Valid nursing license in Massachusetts and a driver’s license.
• Other: Basic computer skills and proficiency in English.
