Job Description

The Medical Social Worker provides support to patients with complex psychosocial needs, assisting with eligibility for social programs and community resources. Responsibilities include crisis intervention, discharge planning, and collaboration with an interdisciplinary team. This full-time position offers a 10-hour workday schedule and includes sign-on and relocation bonuses for qualified candidates.

Qualifications:

• Master’s degree in Social Work and current New Mexico licensure as a Licensed Independent or Clinical Social Worker.
• Five years of hospital social work or discharge planning experience required.
• National Case Management certification preferred.
