Position Summary: As a Physical Therapist (PT), you will assist patients in restoring function, relieving pain, and preventing disability through preventive care, rehabilitation, and treatment for various conditions. You will maintain detailed documentation of patient progress.

Responsibilities:

• Conduct initial evaluations and develop care plans.
• Perform therapy procedures and document patient progress.
• Complete assessments and OASIS data timely.
• Supervise Physical Therapy Assistants.
• Communicate with healthcare providers for coordinated care.

Qualifications:

• Master’s degree in Physical Therapy.
• Active state license and at least one year of experience.
• Valid driver’s license and vehicle for home visits.
