You will maintain detailed documentation of patient progress.
Responsibilities:
- Conduct initial evaluations and develop care plans.
- Perform therapy procedures and document patient progress.
- Complete assessments and OASIS data timely.
- Supervise Physical Therapy Assistants.
- Communicate with healthcare providers for coordinated care.
Qualifications:
- Master’s degree in Physical Therapy.
- Active state license and at least one year of experience.
- Valid driver’s license and vehicle for home visits.
