Position: Physical Therapist (PT)
Sign-on Bonus: $15,000!

Summary:
The Physical Therapist will provide care to patients in the south valley, focusing on restoring function, relieving pain, and preventing disability. Responsibilities include conducting evaluations, developing treatment plans, performing therapy procedures, and documenting patient progress. The role requires effective communication with healthcare providers and supervision of Physical Therapy Assistants.

Qualifications:
– Master’s degree in Physical Therapy
– Active state license
– Minimum one year of experience
– Valid driver’s license and vehicle for home visits
– Experience with Home Health documentation
