Responsibilities:

• Evaluate patients’ functional abilities and create individualized care plans.
• Provide therapy interventions in various settings.
• Implement treatment plans to enhance strength, mobility, and daily living activities.
• Educate patients and families on adaptive techniques.
• Collaborate with interdisciplinary teams for effective patient care.
• Maintain accurate documentation and conduct home safety assessments.
• Monitor patient progress and adjust treatment goals as necessary.
