Job Description

We are seeking a Home Health Physical Therapist (PT) to join our team in the Virginia Beach area, providing direct patient care and collaborating with an interdisciplinary team to enhance patient and family experiences.

Responsibilities:

• Deliver physical therapy services per physician’s plan of care.
• Initiate therapy programs and instruct family/caregivers.
• Prepare clinical summaries and participate in discharge planning.
• Supervise Physical Therapy Assistants and ensure regulatory compliance.

Qualifications:

• Graduate of a CAPTE-approved physical therapist program with current state licensure.
• Minimum of 2 years of adult physical therapy experience; home health preferred.
• Commitment to clinical excellence and documentation.
