Responsibilities:

• Provide physical therapy services per physician’s plan of care.
• Initiate therapy programs and instruct family members as needed.
• Prepare clinical summaries and participate in discharge planning.
• Supervise Physical Therapy Assistants and ensure compliance with regulations.

Qualifications:

• Graduate of a CAPTE-approved physical therapist program with current state licensure.
• 2 years of adult physical therapy experience; home health experience preferred.
• Commitment to clinical excellence and documentation.
