A Physical Therapist is needed for a home health agency serving San Joaquin County and nearby areas. Positions are available full-time or part-time, with mileage reimbursement and paid driving time.

Skills/Requirements:

• Degree in physical therapy from an accredited program
• State licensure as a physical therapist
• Minimum of two years of experience, preferably in home health or hospice
• Strong communication and organizational skills
• Current CPR certification
• Valid driver’s license and insured vehicle

Responsibilities:

• Initiate evaluations and care plans
• Educate patients on prosthetic devices
• Document clinical progress using OASIS
• Participate in care planning and discharge processes
• Consult with families and caregivers
• Provide in-service education for nursing staff
• Engage in interdisciplinary team activities
• Supervise physical therapy assistants
