We are seeking a full-time RN MDS Nurse / Resident Assessment Coordinator (RAC) to join our team. This role is essential for the accurate completion of MDS assessments and coordination of the RAI process, ensuring compliance with regulations and appropriate reimbursement for residents.

Qualifications:

• Graduate of an accredited nursing school; valid RN license
• Three years of long-term care experience preferred
• Experience with MDS/RAI process and case management preferred
